COMMUNITY CENTER CLEANING BID FORM

1. SCOPE OF SERVICES: All services are to e done during the first week of every month unless otherwise stated
Floors: Sweep and Mop every month; Wax every-other month
Front Entry: Sweep and keep organized; Remove snow from front door during winter
Windows: Clean inside and out every couple months or as needed
Bathroom: Sweep and mop floor; clean countertop, and mirror; clean the toilet inside and out every month
General Cleaning: Dust everywhere; Wipe down tables, countertops, stove, & Furniture. Wipe down front of stove, fridge, & microwave as needed. Remove trash. Clean trash cans as needed every month. Cleaning supplies and equipment shall be provided by the Contractor. Trash Bags will be provided by the council.
2. TERM: The term of this Agreement starts on the Commencement Date set forth above and, unless sooner terminated, ends on the Termination Date set forth above. The Council shall have the right, in its sole discretion, to terminate this Agreement on five (5) days' prior written notice to Contractor, and Contractor shall be paid for its Services to the effective date of termination.
3. PAYMENT: Contractor shall submit a monthly invoice to the Council via the Council address or email, for the amounts due under this agreement by no later than the 15th of every month. Subject to the conditions for payment and limitations on liability set forth herein, the Council shall pay Contractor within (10) days after the Councils receipt of an invoice. Invoice is shall be in company letterhead and include a detailed, itemized statement of all charges for which payment is sought, specifying for each date the scope of leaning done. Payments will be made to contractor's business only, no personal names will be accepted. If the Council contests any invoice or portion thereof, the contested part of the invoice shall not be due until the dispute has been resolved.
4. PERFORMANCE: Contractor shall perform all Services diligently and in a good, professional and first class manner, using good quality equipment and workmanship and sufficient trained personnel to complete the service in a safe and timely manner which does not unduly interfere with the operation of the property. Contractor shall obtain and maintain, at its expense and at all times during the term of this agreement, all necessary licenses, permits, training, or other authorizations which may be necessary to perform the service.
5. NO SUBCONTRACTING OR ASSIGNMENT BY CONTRACTOR: Contractor shall not engage subcontractors to perform Services without Council's prior written consent. This a personal service contract with Contractor and is not assignable by Contractor. No references in this agreement to subcontractors shall be construed to authorize their use without Council's prior written consent.
6. INSURANCE: Throughout the term of this Agreement, Contractor shall maintain proper insurance. Contractor must be Licensed and Insured. Contractor shall deliver to the Council a certificate of insurance evidencing that all such coverages are in full force and effective before starting to perform the Service, and if Contractor's insurance shall expire or terminate before the termination date of this Agreement, Contractor shall deliver a new Certificate of Insurance evidencing the new policies of insurance not less than (10) days before the new policies go into effect. All Certificates will be kept on file with the council.
